1. I hereby declare that the audio services provided are carried out at my explicit request and at my own expense. I confirm that no guarantee of any kind of success, particularly in terms of healing or relief, has been promised to me. Furthermore, I explicitly declare that I also approve the fact that no relief or any other improvement of my conditions and illnesses can be expected from the present treatment. Accordingly, the applied method does not constitute a substitute for medical diagnosis and treatment. With this document, I have been informed that I should consult my doctor, psychologist, or psychotherapist for diagnosis and therapy. Complaints of medical background or psychological disorders will not be treated. The activity is limited to scientifically unrecognized methods.

2. I confirm that I do not suffer from any mental illness (particularly schizophrenia, depression, psychoses, etc.). I also do not suffer from diabetes or seizure disorders such as epilepsy or asthma. I am in good health condition, and hypnosis is performed at my own risk. There is no pregnancy.

3. The liability of the hypnotist is mutually and to the fullest extent possible limited, as far as legally permissible in individual cases. No guarantee is provided for successful hypnosis. The success depends on the personal and mental constitution of the client.

4. I have carefully read, understood, and fully agree with the content of the consent declaration mentioned above.

5. I am aware that the acquisition of the offered audio files is not subject to exchange or refund rights.
